Service Authorization, Liability Waiver & Release — JC Mobile Mechanics, Johnson City, TN, (423) 631-3841

1. Authorization & test drive. I authorize JC Mobile Mechanics (“Company”) to perform the service listed above on my vehicle, including operating and test-driving it as reasonably needed for diagnosis and to verify the work.
2. Mobile service conditions. I understand the work is performed at or near the location above — roadside, parking lot, driveway, or similar — where conditions such as traffic, weather, ground, lighting, and space are outside the mechanic’s control, and I assume the risks inherent in mobile and roadside repair.
3. Release of liability. To the fullest extent allowed by Tennessee law, I release and discharge JC Mobile Mechanics, the mechanic, and their helpers from all claims, demands, damages, and liability arising from or related to the services, except where caused by gross negligence or willful misconduct.
4. Pre-existing conditions. The Company is not responsible for pre-existing conditions, worn or hidden defects, prior repairs by others, or failures unrelated to the work performed, whether they appear before, during, or after service.
5. Parts & warranty. Parts I supply are installed without any warranty whatsoever. No warranty is provided on any part or labor unless it is stated in writing on this form.
6. Vehicle & property. The Company is not responsible for loss of or damage to my vehicle, or to personal property left in it, caused by fire, theft, weather, vandalism, or other events beyond the mechanic’s control while at the service location.
7. Transportation. If the mechanic transports me in any vehicle, or moves, drives, or transports my vehicle, I voluntarily accept that transportation, I assume the risks of riding in or having my vehicle transported, and I release JC Mobile Mechanics from liability for personal injury or property damage during that transportation, except for gross negligence or willful misconduct.
8. Payment. I agree to pay for the services as quoted and authorized, including any additional work I approve before it is done.
9. License photo consent. I consent to a photo of my driver’s license being taken and kept on file with this signed form for identification and record-keeping.
10. Indemnification. I agree to indemnify and hold harmless JC Mobile Mechanics from third-party claims, damages, and expenses arising from the services or transportation described in this form.
11. General. If any part of this form is found unenforceable, the rest still applies. Tennessee law governs this form. This form is the entire agreement for this service visit.
